My 'upload attachment' button disappeared from my portal form. Before this week,  every things are prefect and I can upload attachments from portal form to entity Note. However I just find my  'upload attachment' button disappeared from all of my portal form. I am sure I did not change anythings. If you launched any update??

BTW, some tutorials said I should set permission, but it still does not work.

    @Anonymous - This is occurring because the entity form on which you have enabled upload attachment does not have entity permissions enabled. You will have to enable entity permissions on the entity form and ensure this entity permission associated with the corresponding role that provides your user the ability to append notes to the parent entity

    This is per the recent portal update release 9.1.11.22

    If a user doesn't have permission to save attachments no warning is shown and the form is submitted without attached file. After this release, if the user doesn't have permission to save attachments (permissions on annotation entity), then the attach file option will not be shown in Entity/Web forms.

    @sandeepd  I follow the https://crmhub.blogspot.com/2018/09/upload-document-working-with-notes-upload-download-Edit-Delete-document-from-D365-portal.html

    and added permission, but I still did not see that "upload file" button in either edit or insert mode.

    the images attached show what I did in my portal. Do I miss to set something???

    thanks for your reply.

    Thanks everyone, I got the solution finally. We have to set entity permission for both note and customer entity
