The flow completes one pass but does not loop. It creates a file with the correct details set out by the flow.

The error "An action failed. No dependent actions succeeded." is saying that there is one action failed in the Condition control. To trouble shoot you need to go into the Condition control and find the failed action as you did, but one more step you need to "click to download" and see the detailed errors for the specific action:
For your scenario, I would recommend add a delay control with 5-15 seconds:

I found the issue eventually. It was the get row - The bot needed a unique identifier which wasn't present in the excel table.
