What are the requirements for fully testing approval workflows within an organization? What does the individual service user(account) need? Power Automate App, MS 365 License ....?

If you (and I mean all users from your organization implied in appoval process) have Microsoft 365 licences you can try at full capacity for approval workflows as Approvals connector is a standard one.
There are two types of connectors inside PowerAutomate/PowerApss - standard connector and premium connector. The main differences between this categories is that to access the first one you only need a Microsoft 365 licence and to access the second one you need a PowerAutomate/PowerApps licence.
Here is a list with Office 365 plans (except the Microsoft 365 Apps bussiness / entreprise) that give you access to Power Automate/Power Apps using the standard connectors.
If you want to access data using premium connector (here is a large list with this connectors) you'll need PowerAutomate / PowerApps licence (for you as developer and for each user accesing the app / flow).
