partially working when library or list items

Im not sure why this flow is not working consistently.Sometimes,its working as expected i.e., updating list item and creating subfolders and adding attachments .Mostly its ignores update list items action but creating subfolders attachments .overall, this flows works partially most of the times for 2 action items.

Action1:update list item

Action2:create sub folders and add attachment to the specified folders
