I'm trying to make a flow that takes a file from a library and creates a new item in a SP list and adds that file as an attachment to the new list item

Hello, 

Please help. I am getting an error with my flow and not sure what I am doing wrong. The error I am getting is Item does not exist. It may have been deleted by another user.
