I have an approval flow that requires a document to be approved.
The link is included for the end user to view the document and approve reject accordingly.
Majority of our users have moved across to new Outlook bar a few.
The new Outlook does not attach any links to the document and the users are forced to approve via Teams - not all user work in Teams as much and prefer to use Outlook

I tried to replicate your issue by using both "Start and wait for approval" and "Create an approval" actions and passing the document link in the Item Link field, It did not work.
But you can use the Details field and Markdown for the link and it works. Here's what you will put in the Details field: [Text](URL)
You can replace the manual link with Dynamic Content as well.
