how to make amendment on existing system .

Good day I ope someone can help me how to sort a problem we have power apps has been made 3 years ago and worked reasonable well. Now in the order management app we have alight problem the first paid has been pilfered. and is only 5 lines and have recreated. it was made in a model driven management We just want to fee the form back in position but cannot get it done properly. i can give more details but think it must be something simple i can mail or ap screenshots. if questions let me know here thank you.
ofcourse we can sent more print screens but it look simple but cannot get it done.

    Thanks for sharing the screenshot. Looking at the form designer, it appears that the form itself is still there and the fields are available in the Table columns pane on the left.

    Based on what I'm seeing, this doesn't look like the table or data has been lost. It looks more like the form layout was modified or recreated and the fields need to be added back to the form. 

    In the modern form designer, try:
    1. Open the main form you're editing.
    2. Locate the missing fields in the Table columns pane.
    3. Drag the fields back onto the form where they should appear.
    4. Save and Publish the form.
    5. Refresh the model-driven app and verify the changes.
    One thing I'd like to confirm: when you say "the first page has been pilfered and is only 5 lines", do you mean:
    • The form is displaying only a few fields (like in the screenshot), or
    • An entire tab/section is missing from the form?
    If you can share a screenshot of what the form should look like versus what it looks like now, the community can provide more specific guidance. Fortunately, from the screenshot, it appears the form is still editable and the columns are available, which is usually a good sign that the issue is with the form layout rather than the underlying data
