Issue when creating record with lookup columns

I am trying to create a new record in a table called "Referrals" where there are three lookup columns - Account, Contact and Enquiries. I am using the guid value extracted from the relevant tables to use as the input value in the Referrals table columns, however it keeps failing on this error -

I have tried hard coding the values in the lookup columns as well as dynamic content.
 
Is there anything glaringly wrong with this?

    Lookup values for account and contact are good, please check for enquiries table by using "Copy set name"

    Fixed by copying £set name" as suggested by @vejai and removing "/" from before the entity set name.
