Approval Flow Updating Approval Status Column Working, However.....

I have a simple flow configured to approve new items added to a list.  The idea is that the admin of the list would approve items or soft reject items which require additional information or clarification.

 

After seeing mixed information about whether Flow supports this feature, I was able to get it to work using folder metadata and ETags.  After setting it up and testing, it does work as expected for both accept/reject functionality.  

 

However, I'm getting an error message stating the approval action failed for both accept and reject.  The actual message is:

Start an Approval action failed. Please check the values assigned for the action.

 

I've tried tweaking various settings and moving the get metadata instruction to various places in the flow, but I always get the same answer.  When running a test, it says "skipped" at the Condition box.  

 

Here is what the flow looks like.  Any ideas?

    Could you please share a screenshot of the configuration of the Start and wait for an approval action?

    Could you please share a screenshot of the run history of the flow when the Start and wait for an approval action run failed?

    Please choose the Outcome dynamic content in stead of the Response dynamic content from the Start and wait for an approval action in the first value of the Condition.

    I ended up redoing the flow from scratch.  It now works as expected, but I still get the error message.  The screens are below:
