MS Form info not populating in ADO work item

Hello,
I have created a flow and successfully created a work item; however, none of the info from the MS form is populating in the work item (screenshot attached). I've read through the forum and saw a similar report in 2022 but wasn't clear on the resolution. Appreciate any insights.

I only have one solution where I am creating Azure DevOps items. From your screen shot, it looks like you have it configured correctly. In my solution I am formatting the description with HTML. You could try that to see if it makes any difference. Here is how I have my flow configured, maybe you'll see something I didn't see in your action.

My values come from a SharePoint list, but I cannot imagine the values from a Forms response would not work as well.

Thank you David for your response. After lots of searching, I finally found that one of the fields require an integer and my form had text which apparently would not allow any information to pass through. Appreciate your advice!
