MS Forms - When new response is created - running twice

We have a flow that runs when a MS Form is submitted.  Been running fine for years.
 
Today we noticed this has been running twice for every form submit for at least 30 days (Can only go back so far)
 
This is a basic flow, when a new response is submitted... 
 
Not really a way I can think of to make this run twice, seems like form is only submitted once?  Did microsoft change something?  If so in a flow how would you ensure it only runs once?

    I checked a few of my flows based on MS Forms and all are only triggering once per response. Have you made any changes to the default settings of the trigger? If so, please show them. Otherwise, everything looks correct in your flow, and I don't see why it should trigger twice for each submission. I think you will need to submit a ticket directly to Microsoft via your tenant's admin center so Microsoft can investigate.
     
    Unless someone else can spot something with what you provided that would cause it to trigger multiple times, I don't think you'll get a resolution to this in the community forums.
     
    I would suggest looking at the flow runs and make sure you see duplicate IDs from the trigger.

    I'll check settings I dont think anything has changed, but I'll check in a bit.  What settings would cause it to execute twice though, seems like it is comign fromt he MS Form side, triggering twice for some reason, yes I was thinking this may be a MS thing...

    Offhand I don't know of any settings to the trigger that would make it trigger multiple times, but then I never change the default settings of the action when a new response is submitted. This is why I asked if you changed anything from the defaults, as maybe that is the cause of the issue.

    Never saw this issue before: I'm wondering if this can be some caching issue. 
     
    You could try to create a clone of this flow and turn off the original one. If it works fine, you can stay with this second version.
