Since Create_an_approval fails, the process stops before the email is sent.
Common Causes
Expired or broken Power Automate connections (Approvals, Outlook, Teams).
The maker account is disabled, deleted, or unlicensed.
The service account running the CoE Starter Kit flow is invalid or missing permissions.
Issues with the Approvals connector.
Recent DLP, security, or environment changes.
Open the failed flow run and inspect the Create_an_approval action details. The detailed error message will reveal the actual cause (e.g., invalid user,
authentication failure, licensing issue, or approval service error).
The issue is not with the app deletion process itself—the failure occurs before the maker notification is created. Resolving the Create Approval error should
restore the inactivity notification emails and allow makers to decide whether to keep or archive their apps.
