Blank Items created in SharePoint Lookup list???

I have a Primary sharePoint list and then a lookup list with a list of values that is shown in the Primary list.

 

When I create new items in the Primary list via my Power App there appears to be random blank items created in the lookup list... Does anyone know why this would occur???

 

I hope someone can help?

    Please show code and images. No way to know why its happening.

     

    How do rows normally get into the LookUp list?
