Approval flow not initialising (XrmInstanceProvisioningIncomplete error)

I created a very simple flow where a document needs to be approved. The trigger is set when a file is created/modified. I get following error:

Anyone else experienced this issue and knows how to solve it?

    I am having the same issue with a Flow that has been successfully deployed to many other tenants without issue.

    Same issue here. It has been working fine for a year. It suddently stopped working a week ago and all approval history disappeared from the approvers' Flow portal.

    Hi @v-monli-msft, We have a similar error.  Was there a resolution for this?

    Would you please share the detailed configuration of your flow? Including fields in trigger and action. I just test with a simple one and got the same result"
