How to get Answers and Question from a Form to a list.

Hello, new to Power Automate.

Simple task, in the flow Create Item, I want to also include the question on the list I am sending the answers to.

I created a list with columns to have only one single answer appear. But I need every question to appear too. because they are all unique. It will look like this:

QUESTION 1 Answer 1

QUESTION 2 Answer 2

etc...

Is this possible? that's all the information I need but I am not sure how to go about it.

Doesn't matter at what step the action is added

I will give it a try. I believe I was doing exactly what you show there. Should it matter what step in my flow this is?
I'm not ready to give up.

I was using the add custom option and still was not working. From there nothing else works because it cannot find the adress. I am not sure how to go about that.

@EdgarHR1, I think you might be pasting the url.

Click the dropdown, select custom, and then add the url.
