Survey form with multiple Radio Buttons

Hi All,

I am trying to create survey form as per the below attached screenshot. For each question in the form, user need to submit answers in each radio button. And the condition is if user selects 2nd, 3rd and 4th answer in the radio button, for each answer Comment Box should be visible to user to add the comments.

Please suugest how we can achieve this requirement in PowerApps.

hey @Arpit28

is this form part of a bigger solution or why did u choose power apps for it? Maybe Microsoft Forms would be faster in easier to set up. Have u thought about it?

Hi @mmbr1606 ,

This was the requirement from management regarding PowerApp. The screenshot which I have added is for a single question in the form, There are 5-6 questions with the similar view containing Radio Buttons to select the answer. Will it be possible to develop like this in PowerApps?
