Can anyone help with this? It's driving me nuts! In the variables tab it clearly shows that this variable has a value but for some reason it's reading it as blank in this formula.

It seems that the variable reference in your Filter is linked to a context variable, whereas the variable in the left pane is a global variable called 'ctxProviderNPI'. It is best to prefix global variables with gbl instead to avoid confusion.
In this scenario can you confirm that there is no UpdateContext() function used in that screen that updates the ctxProviderNPI. I expect that you may have both a global and context variable with the same name, the latter having no value.

Maybe it is a crash...
Try to refer to the formula you set for the global variable directly.
