Hello,
I have a Microsoft form that store information into a SQL table but now it would be great if I could retrive some information from the database (based on the email of the person filling the form) such as last appointments (from sql database) and in the form in one of the questions the user could select them.
Is thewre any way?

Unfortunately, linking to a database or dynamically passing on data in a Microsoft forms is unavailable. If the respondents to the form are internal (within the organization), you could spin up a Power App and implement the functionality there.
