Looking for Suggestions - Employee Scorecards

Hey There - apologies in advance if this isn't in the right place (hopefully someone can guide me to where would be the right place if not) and for the vague examples I may give (covering myself under a company and client NDA).  You all have given me solid answers on the how-to's in the past, so I'm hoping that maybe someone can offer me a place to start in finding a way to accomplish the creation of some sort of employee scorecard that captures certain items.

I know I could do this via Dynamics; I've done that before.  Unfortunately, Dynamics money isn't really something that the company is looking to put out right now.  And I could do it via Access, but there'd be limited automation.   I've got a little experience with the bulk of Power Platform products, and can figure out pretty much anything as far as the "how-to" so I don't need anyone to guide me through the creation; just something that would give me a few trees to bark up as to how I could incorporate it all.

We have a heavy rotation of employees; usually around 3,000 at a time, but the turnover cycles in/out between 100-200 a week, and if they aren't changing, their manager is.  There's a feed that comes out every morning I can extract / update that information from with an Employee ID being a constant on the record.  But I'd like to be able to tie a couple of pieces to that record.

- Each employee must have a monthly meeting with their manager that covers certain components.  Those components being discussed has to be documented.  Right now, we're using Forms to record these meetings, which save to a Shared Workbook, then manually Pivoting / VLOOKUPing the information against the extract for the day.

- Employees that commit specific security infractions need to have them recorded along with specific details for investigation by internal security.  There's a Form set up for this too, with branching for the various types of incidents that occur.  This all routes into a Sharepoint where the IS team can assign the records, update the status, choose the closure resolution, etc.

- Any employees that move addresses with company issued equipment must first have it approved by management and security. This is fed through a form to another Sharepoint list.

- Annual training and NDA signing is required.  We track both of these manually currently, but have implemented a Forms to SP process for the NDA signing so they are named properly and we can just compare against the employee extract.

I'd like folks to be able to pull up a "Snapshot" of an employee at any time with all of the above somehow.  It doesn't necessarily need to be "up to the second" - it can go on a once a day refresh.  I'd also like to have a summary by the aligned manager of their meeting progress.  Ideally, I'd like it to be something that doesn't require a per person license, as the personnel, even at supervisor level, is constantly changing and it's challenging enough to get them the right access based on that.  Plus the cost factor, of course.  I don't mind going several layers deep in creating things to make it work, I'm just not really sure what the most effective would be.

I've attached some examples of what a rough example of each would look like; just something I threw together in Excel.  I appreciate any guidance y'all may be able to offer!
