There was a Problem Reaching the App

Hi All,
 
I have developed few applications and all were working fine but from last few days, many users are getting below error notification and some users are not able to use submit form function , it says network error while patching.
 
While some users are not able to see complete items from sharepoint as there are not filter enabled at the moment.
 
 
Can someone please recommend/suggest if this is something to do with authentication or firewall issues ?
 
How can we resolve this ?

    There's definitely a problem with Patch. Is the Patch working for you? If yes, then this could be a Permissions issue. Please make sure the users of this app have the required read and edit permissions in your SharePoint.  

    How many items are you displaying in the app?
    Is the app working fine for you?
