Power Apps Not Caching Properly

Hi:
About two weeks back when we would, from a SharePoint list, click on Integrate -> Customize from, the system would load the dev editor and we could attach to the SharePoint list, and implement other connectors.  We would then lay out a custom form and publish it back to the list.  When clicking on the NEW+ button the default edit form loads NOT the customize power apps form that was previously saved.  No errors showed on the Publish function.  When we go into LIST Settings and then FORMS and click on versions and then PLAY the correct customized form loads and operates as expected.  Every time we go into the new dev editor it acts like it is the first time we have been in the editor, prompts with all of the pop ups and when you click on DO NOT SHOW the next time they come up.
 
We have older forms done this way that work on different lists and they still work as expected.  have not attempted to editor the older forms in case they exhibit the same behavior.
 
Forms below show this, and we can reproduce this with a new list.  So this is repeatable.
 
Any thoughts on how we can resolve this?  Have opened a case but so far, no answers.

    Hi:

    Finally got this to work.  Found a couple of articles that suggested adding another control onto the custom form and switching between default, Classic and New then re synching.  Did that several times and finally worked.  Also made sure permissions on the list were high enough.  Still think this should not require those steps.  But grateful it works now.
