Any Suggestions how I can troubleshoot this issue further? All Table Permissions have been granted to the entity and any lookup on the entity.. So how can I get to know further what might be causing the issue?

Pls confirm is it related to notes/ attachments or SharePoint integration?

If it is just entity lists make sure you go to contacts > related > web roles and add appropriate web roles.

It is currently not related to notes/attachments or sharepoint integration... I want to enable sharepoint integration but i am removing the grid from the Basic form to troubleshoot.

Web roles are added to my contact.

I found the issue is pretty awkward the issue that on the entity list itself I was not Enabling Entity Permissions and I had to enable it so the basic form works without the permission issue.

So the Trick is Enable Entity Permissions on the Entity List
