Customized SharePoint Form using 2 screens requires powerapps license?

Good day,
I was wondering if anyone could point me in to why I am receiving this issue. Our team is working on a relatively straightforward / simple form, and we opted to customize the form with PowerApps.

However, we have users getting a "Your Power Apps trial has expired".

All I have read is that as long as the form/app is not using premium connections, we should be fine.

The form has 2 screens (New & Edit) and 3 connections, all to SharePoint lists.

We do run an automated workflow that grabs data, places it into a word document, and attaches to the SharePoint list, but at no point does it interact with the application.

We have done other forms before that these same users do not receive an error with.

Just curious where/what to look for as we are stumped...?

    Did you check all your datasources and connectors used in the app? Maybe there is a premium connector hiding?

    What license does the users have? Does it include PowerApps or is it turned off for them?

    There is only 3 Datasources, which were all Sharepoint lists that we generated.  Unless I'm misunderstanding (I'm newer to trying to understand the license aspect and connector aspect so it's possible) but when adding the SharePoint lists, it just showed the standard (no premium icon) connector.  I've attached a picture of the datasources.

    I can reach out to our SharePoint Admin to see what license the user has, but if they can view 1 'customized' form, shouldn't they be able to do it to this one?  To be fair, this one had a lot more work done in the behind the scenes (2 screens, variables, multiple dropdown options, patching, etc. ) as compared to just dropping all the Fields into a form and making it look pretty.  Shouldn't the same license work for both?
