Hello,
I have created a SharePoint List customized PowerApps form. Everything works fine for me, however, I couldn't see the app in my phone under "All Apps". How can I view my app in my phone. Can anyone please help me here? Really appreciate your help, Thank you!

I think you have to use a browser to view the SharePoint list and you'll see your customized form.
If you create an App from make.powerapps, you can see the app in you powerapps application or browse to it using the URL you can grab from the app details.

Problem is this app will be used by technicians in remote site and they have limited knowledge about browser usage. And we dont want them to go look for this link/Qr code every time. We would basically install power apps on their phone and would train them how to access the app through phone.Yes, I can view this App using browser and using the link/QR code but I would like to view this app under "All Apps" on their phone/my phone. I couldn't see the app in my phone too.
And I didn't create this App from make.powerapps, I've attached a screenshot how i created the app from Sharepoint list. May be that's the reason I couldn't see the app under "All Apps" in 'Power apps' App.

In order to make the app appear in the Power Apps mobile app you will have to create a standalone Power App built on your SharePoint list as a datasource - not a customized form. These Forms are available through the browser experience but will not appear in the Power Apps mobile app.
Should you be looking for a simple data entry and viewing experience - to avoid a lot of additional development you could opt for the 'Start with data' option in the make.powerapps.com home screen. The new generated app template is also fully responsive which is a big advantage.
Although I am not aware of the full business case, SharePoint customized forms does not look like the go-to approach to meet your requirements. You could use the URL and display it in a browser as stated above but I would recommend using a standalone app using the SharePoint connector.
