I am having trouble with my model-driven app that I can't seem to fix permanently. We have set up user mailboxes for outgoing and appointments only, which allows us to send appointments we create to a shared calendar. Everything works fine for a few weeks, but then the mailboxes stop sending mail again. To fix this, I have to test and enable the mailboxes. The main issue is that there are no alerts or errors on the mailboxes or mail server profile to indicate what the problem is. The mailboxes are listed under the “system-disabled mailboxes that previously synced properly” view. Additionally, the Server-Side Synchronization Monitoring dashboard would show the mailboxes as healthy and running, even when they are not. What could the problem be?

I'd agree with @shashankbhide sounds like you had it all setup and working, then something stepped in and broke it. Possibly something to lookup as part of Exchange, but I'd raise with Microsoft.
looks like you've configured correctly, because you are able to send/receive emails, I'd raise a support ticket.
