Unable to Save Custom Page to Model Driven App

I'm running into an issue where I can't save a custom page on a model driven app.

I'm able to save and publish without it throwing any errors, but when I "play" the model driven app OR come back to edit this same model driven app, it's not there. The canvas app that I'm trying to embed works on its own just fine.

Looking at your question and at your images - I believe you are in a preview that us none employees don't have access to.. Custom pages aren't due to be available until December https://docs.microsoft.com/en-us/power-platform-release-plan/2020wave2/power-apps/makers-add-custom-page-model-driven-app-designer
and if you are in teams I don't think project oakdale is in public preview yet
