Groupby, distinct and filtering gallery

Hi, 
 
I want to create a gallery in PowerApps using groupby, filter and distinct all together but not sure how.
 
I have a set of data where column 'Name' and Action are a choice column, the rest are text.
 
Name (choice) Description (Text) Issues (Text) Action Y/N (Choice)
Company A Product 1 Issue 1 Y
Company A Product 2 Issue 1 Y
Company A Product 2 Issue 2 Y
Company A Product 2 Issue 3 Y
Company B Product 3 Issue 1 N
Company C Product 4 Issue 1 Y
 
I want to group by the column 'Name' and 'Description', while showing both data in the gallery and filtering those Action with 'Y' only.
 
Company A Product 1
Company A Product 2
Company C Product 4
 
 
Anyone has any idea?
