display or pull my manager name from sharepoint list

Hi all, I have a form.. i want to display my Manager name

 

i have sharepoint list consit all details of my company employee with various department & designation that have name, designation & department (sharepoint list name is employee) - is like reference table for employee details

 

Logic:

if Office365Users.MyProfileV2().department = sharepoint list (employee.department) && employee.designation = "MANAGER" will display my manager name

 

Anybody can help to achieve my logic above?

 

The Hierarchy:

ME > SUPERVISOR > MANAGER

 

The objective is I want to display my manager name from employee sharepoint list before submit form.

    Could you tell me:

    Is there a SharePoint list named "dmployee" something like this:
    Employee

    Title department designation
    Lee Dept1 MANAGER
    Max Dept1 SUPERVISOR

    If so the formula should be:

    Lookup(
     Employee,
     department=Office365Users.MyProfileV2().department &&
     designation="MANAGER"
    ).Title

    @v-bofeng-msftyes...totaly exactly as my situation..i have use that formula and success... but it appear delegate (yellow) warning icon on that input..maybe dua my sharepoint consist 1000++ record staff..
