adding additional data card in Form

Hi I am trying to create a form where few of the fields could be added more than once (situational) by click of button. (in my case i want to add issues and photos multiple times). Every time when user wants to add new issues, it should add new fields by the click of "add issues button" while not resetting the whole  form. The fields which are already filled (tittle, name of the shop, date of the visit, person  contacted) should retain the data unchanged. And when the form is submitted, , it should update sharepoint list as separate records for every new issues added. 
The idea here is user should be able to add multiple (selected) entries without having to fill all the fields over and over again. At the same time sharepoint should get data as a new record for each additional entry. 
 
 
if some one can help that would be greatly appreciated.

    Your Issues and Images need to be in a "child" database with a one-to-many relationship between "parent" and "child".
    Then you can save multiple entries of Issues and/or Images against one entry in the main table.
    If you've not done this before, read up on one-to-many relationships in database structures (it's not a scary as it sounds)

    You can do this by using a collection.
    1. Create a blank collection with necessary fields.
    2. Add button so that on click, it adds a new item to the collection
    3. When submitting to SharePoint, you will need to map the columns in the collection to SharePoint.
    Here is an example.
    Collection: 
    ClearCollect(collectionData,{key1:Blank(),key2:Blank()})
    Gallery with two input controls:
    Add item button onselect property:
    Patch(collectionData,Defaults(collectionData),{key1:Blank(),key2:Blank()})
    Upon clicking the button.
