What is the approach to embed a power app form inside our sharepoint modern page

I am trying to achieve the following:-

1) I have a custom SharePoint list with 2 fields (Title & Comment).

2) I want to embed the list create form inside a modern SharePoint page, where the embed page will show the 2 fields + a submit button, as follow:-

so i am not sure how i can achieve this?

i know that i can embed a powerapp inside a SharePoint modern page as follow:-

but how i need to build the powerapp? can i embed a customized form? or i need to build a pure app?

    If you are embedding a PowerApp with the iframe or PowerApps preview webpart, you will need to create your app separately.  You will not be able to embed a customized form.

     

    Also...consider parameters that you might need for your App.  If you are just entering in new items through the app, then there is no differentiation between the above methods.  If you are planning to Edit items in it, then you will most likely need to Pass the information to your app in order for the app to display the correct record.  In that case, use the iframe method as the PowerApps preview webpart does not support passing parameters.
