Need help creating an IF statement with a choice in form

I was hoping someone could help with something. I am trying to create an IF statement within a form choice. I need that if when a form is submitted, if a certain choice is selected that it triggers a flow. The List the form (form1) pulls from is 'X -Ray safety checks' with that column simply called 'issues'. I need when 'Yes' is selected as a choice that it would trigger the email flow but i dont even know where to being with it.

Can anyone help?

OnSuccess of the form (assuming here Issues is a Choice column)

If( Self.LastSubmit.Issues.Value = "Yes", FlowName.Run( . . . .
