Ask- Registeration form but only change title and date time ..

I like to find out what is the best method to create a standard registeration form that will capture staff email, department time of register and check in and all regosteration info into SP list

The form can be use for various events everytime whereby we just change the event title, date , and time with venue.. shld this be a SP list??

Any sugguestions? THANKS
