PTO Bank/Request/Approval via Teams & PVA

All - I'm trying to recreate a way to request PTO with approvers through PVA via Teams - like currently capable with Dynamics 365 HR and the teams integration (pics attached).

 

I'd like it to be dynamic in a way that it will show the end user a pick list with values (reasons for time off ) along with a calendar to select the days away. This solution would also house the remaining time an individual would have the ability to take. 

 

Any help or thoughts on how to achieve this would be much appreciated.

    Bumping this to see if someone can advise on best approach to achieve this functionality within teams. First thought was to create a approvals via SharePoint then flow via SharePoint that can be accessed in PVA/Teams. Any recommendations would be most appreciated.
