Unable to Show the User Input Data Fields on My Form Screen in my Canvas App

Hi Power Apps Experts,
 
I am a beginner to Power Apps and just tried a create a 4 page canvas app in which i have 4 icons in the bottom navigation although its not supported in power apps i just created my own bottom nav bar. there is an option which shows a form and when the user click on it he goes to a form screen where I have have given my form. then after that the form is not showinng where as i have already created my data as well and also i have inserted multiple fields in there where user can put his/her data 
 
BUT
 
here is the real problem 
When i play the app and go to the form screen it does not show anything i mean anything. not even the fields etc the screen is black
 
Please guide me on this issue and tell me where am I making a mistake

    It sounds like your form is not rendering properly in the app due to a configuration issue. Check out
    • Data Source Issues:Incorrect data source or disconnected data source.
    • Item Property Misconfiguration: Item is null or incorrectly configured.
    • Form Visibility: The form or its fields have Visible set to false.
    • Screen Navigation: Improper navigation logic causing the screen to load without context.

    Hi,
     
    When you click on the navbar button, you need to set the OnSelect property of the button to NewForm(FormName) to open the form in New Mode.

    For example, if you have an "Add Ticket" button in the navigation, set the OnSelect property as follows:

                    OnSelect = NewForm(Form1); Navigate(TicketScreen)

    Here Form1 is your Form name and TicketScreen is screen name which you want to open.
                   
    This will ensure that when you click the "Add Ticket " button, the form will be opened in the correct mode for creating a new record.
