For a QC/inspection app we want to develop, the operator needs to take a picture of an object and make some drawings on the image he took. Is this possible with the pen input? I cannot seem to find an option to set a background image for that control.

Do you want to set a background image for the Pen Input control?
The Pen Input control is used to draw, erase, and highlight areas of an image, please check the following article for more details:
If you want to make some drawings on an image using Pen Input control, I afraid that there is no way to achieve your needs within PowerApps.
