First message always returning SystemError

Hello there,

I noticed that my first message after authenticating always fails with a SystemError. Here is the response I get:

Sorry, something unexpected happened. We’re looking into it. Error code: SystemError. Conversation ID: a:1RqxfBPJLBb3F8oRfPmGq2NFPfp1TD5KvEWQ2EL94Nlv4TjT_ClHQyFYp_UeAX1asYxcBz5cNItd5fWhIbtIA1NRI0q5UtWQYbfQTv8ZlxPEXCezdf54YJ-Ewz59qQBjQ. Time (UTC): 6/26/2025 10:42:25 AM.

I see this happening for anyone who authenticates and uses the Copilot Agent. It appears to only be affecting the first response. Everything after it works:

@EM-25061139-0 seems there’s an issue with some connector or flow. Could you post a screenshot of that topic?

I am running into a similar issue, the first question returns an error - "Sorry, something went wrong. Error code: SystemError. Conversation ID: XXXX", If I ask the same question again, I get the response.

The other issue is that I don't see the list of tools after adding the custom connector in the tools tab ("No Tools Available"). I see that copilot is making 2 calls to the MCP server when I hit refresh: "initialize" and "notifications/initialized" but it does not call "tools/list".

I am also hitting this same issue with the first response always returning an error and then on the second response CoPilot is able to call the tools and respond correctly.

hello, there is "hidden" design concern and tricky one :) i had often this matter.

What happen here is

1 you ask you question

2 agent at the same time will do this 2 actions

2.1 try your' flow

2.2 say "stop" i need authentification

Result : "broken"

What to do : force user to connect first with connexion message, etc and then it will work. You'r tool need only one time connexion for very long time.
