I need to create a form that has fields on top like name, date department etc and in the form to have two tables columns that can add new rows of records like excel.

How to patch these table of record back to the same item example if the record is for ID 1?? And how to create the table that is auto expand and add new row ??? the action and notes will be very long text sometime. thanks
