Microsoft List Custom Form Not Resetting in New Item

A strange issue has started to occur with my custom forms built using Power Apps for my MS Lists. It seems to have coincided with the new UI style appearing.
 
When adding a new item when first loading the page, everything works fine. When you save to create the new item with the custom form and then click the button at the top of the list to create another new item, all the fields on the custom form are still populated with the previous record submitted.
 
As a workaround, we are refreshing the page and then going again, which works fine.
 
How do I fix this?

    To fix the issue, add the following to your form's OnSuccess property:
    ResetForm(FormName);
    NewForm(FormName);

    I added
    NewForm(Self)

    to the OnSuccess property and it fixed it for me. I actually found it wasn't the previous record that was showing in the box, but the very first one on the list.

    My code now looks like this
    ResetForm(Self);
    NewForm(Self);
    RequestHide()
