Model-driven apps natively just support CDS as their data source.
One option you have is CDS dataflows. Access is a supported connection for a dataflow, and you can have it incrementally refresh the data in your CDS from Access

Running the access database from an Azure VM will give you better performance as it is closer to the Power Platform.
Embedded canvas apps are an option, if you want to view data from Access embedded in a model-driven form.
My recommendation is to ditch access--move your access database to CDS and access it through a canvas or model-driven app. On premise data gateways and access databases generally are slow performing.
