Activity Table Removed from APP

Hi
 
Somehow someone has removed an activity entity table from an app which caused the app to stop working.
 
Is there anyway to query the dataverse to see when the table was removed from the app and by which user?

    you can check the modified on date of your app and not sure we are able to fetch who removed that table because there is no audit for this but you can raise ticket with  Microsoft and they can check from logs.
