How to create add new form button?

Hello,

I should create Power Apps what could be used when we make changes/additions on our items. Plan is that user can type item numbers and reasons (status) what & how details are changed. For example if we have new item or some existing detail changes, users can fill in the form and submit it. This information would go by email what needs to be changed for items.

Question is, how can I create a button where user can add several lines for several items. I can do this with gallery but what about add new form?

Hi

Yes, this is something similar. However it seems that my Power Apps looks different and problems start when I try to create Patch/Collect code strings. Even if I try to do similar way, I end up having multiple errors
