How to build form to allow multiple of same requests on 1 form

Hello,

 

I would like to build a form to allow multiple of the same requests on 1 single form. It gets tricky since each request might need different fields. 

 

I tried building conditional fields that show upon checkbox selection however that data does not save on the form after submitting. 

 

Does anyone have any suggestions?

    Why dont you use the Patch function? Add all the fields, conditions, etc, etc and after that use Patch function.
