Approvals getting cancelled

Hello, I have been running power automate flows using approvals. But recently I have been experiencing problems where the flow run gets cancelled even when the user presses the 'Approve' button.

I went to the approvals tab in the power automate and the status is reflected as Approved. I am wondering why my approvals are getting cancelled when this is the case. I have attached screenshot below.

Also seen this issue. Approvals were in pending status and a bulk of them just went into a cancelled state. No errors, no warnings, no errors in analytics, nothing for me to troubleshoot. Also it disabled the flow due to to many errors.

This service needs to be rock solid for us to use, please advise.
