We have a vast number of Microsoft SQL Servers at our organization, but they are all of course behind a firewall. How can PowerApps reach these if an app runs from the cloud?

You can access internal SQL Servers by installing the on-premises data gateway. Here is a link to more information.

Do you want to use On-premise SQL Table as data source of your app?
If you want to access On-premise SQL table from your app, you must firstly install an On-premise data gateway within your computer. More details about installing a On-premise data gateway within your computer, please check the following article:
