JavaScript navigation causes Page to require refresh

After clicking a button and accepting a dialog for record generation, the navigation JavaScript functions often causes error and requires page to refresh:

This recently started to happen from the use of the deprecated Xrm.Utility.openEntityForm and the use of 

Xrm.Navigation.openForm. The navigateTo function did this once before as well. It all used to work just fine.

Should we switch all over to navigateTo and pray? Or is this an inconvenience that can otherwise be prevented?

    I have plenty of apps that use such navigation approaches, and I do not see this. Are you sure this is not due to some code you have running on the Target of the navigation?
