I am asking the same question in Azure OpenAI chat playground and in the PVA bot. Sometimes, I'm getting the correct answer in the Azure chat playground but PVA is triggering the FallbackCount. Any idea why it might happen? It is not happening always, but sometimes. Below are my diagram for Fallback and conversational boosting.

@fernandosilva Not exactly. I'm mostly using the default topics. Below are the system topics and Fallback is getting triggered.
Ok, so if you create a topic that is not a system one and replicate the steps that a system topic does or something else, but include trigger phrases, that should work.
@fernandosilva yes, that will help if I had a specific set of questions for where I could add the trigger phrases. But the problem is, it is not consistent and random. Any idea how I can generalize it?

The reason it looks like it works sometimes and not other times is based on whether Generative AI finds an answer or not. The Conversational Boost fires first, but if it can't find an answer, it doesn't stop PVA, which results in the system Fallback topic then getting called.
You can always remove that logic in the Fallback topic where it counts the number of times it can't find an answer if you don't want it to to do that. In essence, the below is what is happening:

As @MattJimison said, you have to compromise the Fallback if you don't want it to happen.
The cons about it, is that you won't have any Fallback anywhere else in the bot when you need it.
That's why I recommended to create a new topic with the same structure and trigger it when you need it.
I am experiencing a similar problem. If I feed my Virtual Agent with a website it does nothing at all. When I chat to my chatbot it triggers the fallback topic rather than going through my website.

As I mentioned above, your fallback topic is triggered anytime GenAI can't find an answer. One suggestion would be to lower the moderation level, as when its set high, you'll get less matches. You could also opt to update the GenAI topic and instead of going to the fallback topic, you could output a message saying you were unable to locate an answer. By doing that alone, you should see that the GenAI topic is in fact firing.
I changed the content moderation level from Maximum to medium and it works reasonably well. Thanks Matt!
