Records are not being created when filling out the customer voice survey form.

According to my project requirements, I need to send the survey form to users whose records were automatically created in the PowerApps entity (msfp_customervoicesurveyresponse). However, recently, records are not being created in the Dataverse when the form is filled out. I have two environments for my project: the production environment is working perfectly fine, but the development environment suddenly started experiencing this issue.

Is the Customer Voice Project that form responses are being submitted for in the dev environment?
