power up maker course lesson 14: update model-driven maintenance app

While on the exercise after i have saved, publish, play, click on maintenance log and select new.
The new page has only two fields 'maintenance log name and owner' where as in the video it 
appears to have other fields like priority, service area, location, etc. Why is mine not showing all 
the other fields?

    if you want to have other fields you would need to add them to the forms.
     
     
    powerup program also has a seperate from built in the learning module as well as calls where u can attend and ask questions.
