Good day

Please assist.

I am tasked to convert our IT request forms to online forms and then develop a process that when users are requesting service, on the PowerApps portal that I have built the form can then be attached in the attachment field in PowerApps.

Here is the section where these form(s) should be appended:

Your help is always appreciated.

what exactly do you want to attach to the attachments control?

I want to attach a converted list item to a PDF document and append it to the main portal that submits project Users should navigate to the Project Request Portal in PowerApps to submit their main request form. For certain services, a specific form is required to support their request. This specific form must be available online (as it is currently in paper format) and submitted along with the main request form. I trust this clarifies the process and the rationale behind it.

    Thanks

Shouldn't be that difficult. Use MS Forms(Online) to retrieve the data for the first request form with Power Automate, create a PDF and store it in a specific folder. Via Mail(Link) you can inform them where to find the pdf after creation.
This PDF can then be attached as an attachment in the Project Request Portal in PowerApps. After this step, you can also have the new project folder created (via Power Automate)
You can also link the forms in the Power Apps. Or refer to it via a screen in the previous step.

But you can also do this only via Power Apps. So depending on how the business wants it...

I hope I have understood your requirements correctly.
