Multiple submission via Form

I'm building app which will be collecting new entry from users to SharePoint list.

All functionality is working well, however during one run of application only one submition is available and form is no longer visible.  Looking for solution which help me fill one form multiple times.

    OnSuccess Property of Form:

    FormMode.New(FormName).

     

    every time you submit the form it will start your form in new mode again.
